Medical Assistant (Remote )

* Conduct outbound and inbound patient calls and schedule appointments
* Support Annual Wellness Visits and care management outreach
* Perform medication reconciliation and update patient information
* Document patient interactions accurately in the EHR
* Prepare charts and support care coordination and follow-up
* Maintain HIPAA compliance and escalate clinical concerns appropriately
* Meet established outreach, scheduling, and documentation targets

Qualifications

* Medical Assistant certification or equivalent healthcare experience required
* Prior healthcare, patient outreach, scheduling, or care coordination experience
* Strong communication, organization, and documentation skills
* Comfortable with high-volume calling and performance metrics
* EHR experience preferred
* Medicare patient experience preferred
* Reliable and self-motivated in a remote environment
